Practice Profile

The Practice has been in existence since the early 1900s. It has been sited at various locations in Formby over the years with the present purpose built premises opening to patients on 1st May 1989 with its new title of "The Village Surgery, Formby"


There are 5 partners, plus a part-time GP and a full complement of staff including a nurse practitioner, a practice nurse, health care assistants and a highly experienced administrative support team.


We offer on-site parking with two spaces reserved for disabled patients (Cars are parked at owner's risk). The building is accessible to disabled patients and we have disabled toilet facilities.


We offer a range of services to our population of approximately 9,200 patients which include chronic disease monitoring, minor surgery and contraception clinics.
