The NHS Health Check is a statutory national programme delivered locally to eligible adults aged 40-74. It is designed to spot early signs of stroke, kidney disease, heart disease, type 2-diabetes or dementia and to help find ways for individuals to lower their risk.
It is primarily offered to people in Sefton in the 40-74 age groups without a pre-existing condition and has been provided by GPs for the last 5 years.
