'Our Service During The Pandemic' Survey
We are inviting all our patients to take part in our new survey to find out what it has been like using services at our GP practice since the start of the COVID-19 pandemic.
Our practice has continued to provide care and support to patients throughout this time, including seeing anyone who needs a face to face appointment.
To keep you and our staff free from COVID-19, all GP practices put in place some safety measures at the start of the pandemic – like asking you to telephone first and not drop-in, or offering telephone or video appointments.
Many of these measures are still in place in all NHS buildings, such as hospitals and clinics because COVID-19 is still circulating. They are also helping us to manage the growing number of patients contacting us for care and support.
We are working with NHS Southport and Formby Clinical Commissioning Group (CCG) to ask for your experiences of services at our practice during this time.
Your views will help us to understand what has worked well, what has not and what will work in the future.
Our survey will be open for six weeks from Monday 22 November 2021 to Tuesday 4 January 2022 and we are asking patients to go directly to www.southportandformbyccg.nhs.uk/GPsurvey
